To help your first appointment go smoothly, please bring a few basic items with you. If you schedule online, Global Smiles Dental may send a text message with a secure link to complete your paperwork before your visit. Completing your forms ahead of time can help speed up check-in, but it is not required. If you prefer to complete paperwork at the office, our team can help when you arrive.
A photo ID
Your dental insurance card, if you have one
Medicaid card, if applicable
A list of current medications
Any recent dental X-rays, if available
Information about your dental concerns, pain, or treatment goals
A parent or legal guardian for patients under 18
Online Forms Before Your Visit
Global Smiles Dental uses NexHealth for online scheduling and digital paperwork. After scheduling, you may receive a text message with a link to complete your forms before your appointment. This helps our team review your information ahead of time and can make your visit faster and easier.
Prefer Paperwork at the Office?
No problem. If you would rather complete forms in person, please arrive a little early so our team can help you get checked in before your appointment.
